In this episode, we're discussing a paper called "Time Perception in Children with Attention Deficit Hyperactivity Disorder: Does Task Matter? A Meta-Analysis Study." This study looks at how children with ADHD process the passage of time across different senses and experimental conditions. It investigates the exact mechanics behind "time blindness," confirming that our internal tracking systems don't just fail universally; they experience a predictable collapse the longer an interval drags out—specifically stretching past the five-second mark. We’re going to get into all of this because it’s fascinating how relevant this really is.
Skye Waterson: I’m excited to get into it! They looked at several different aspects of time perception as part of this. Before we even get into the meta-analysis, I wonder if we should talk about the idea of these different aspects. They discussed time production, time reproduction, and motor timing, but a lot of people don’t even realize there are different "senses" of time. I think most people know about time blindness by now, but the idea of time reproduction itself is quite specific.
William Curb: Yeah, because we often think of time as a singular thing—just "this is what time is." But how we use that time is very different depending on the condition. I don’t have the specific conditions written right here in my notes; could you go over those?
Skye Waterson: Definitely. The key piece is that when we talk about time, we often think, "Okay, I learned to tell time, so now I have a full grasp of it." We think of it as just "What time is it?" But really, it’s a variety of things. For example, there is "time production," which is how long it takes to do something. Then there is "time reproduction"—if something takes a certain amount of time and you aren't using a stopwatch, can you accurately recreate that same duration? Finally, there is "motor timing," like if I am tapping a rhythm, can you copy that and maintain the pace?
William Curb: Those are the main ones they covered. It’s all about that perception layer. When we say "time blindness is not perceiving time," we have to ask what that means. Is it about being able to reproduce it verbally? Is it physical? Or is it just thinking through how long something took and trying to be accurate?
Skye Waterson: Exactly. A lot of times, people aren't aware that time is an entire segment of executive functioning. When it’s missing, it affects more than just being late; it affects how you perform tasks. You might assume a task will take 20 minutes when it really takes three hours. While that can be a working memory problem or a failure to break down the task, it might also be a time reproduction error. I’ve found while researching this over the past few years that it’s a relatively underexplored area in terms of general understanding.
William Curb: I think it goes the other way, too. People look at a task and their brain assigns an additional "load" to it because it’s boring or they don’t want to do it. Their brain says, "Don’t put your coat away; that takes too long." In reality, it takes two seconds, but throwing it on the floor takes zero seconds, so the brain chooses the latter!
Skye Waterson: I am so guilty of that! So, let’s look at what they actually did in this study. This was a meta-analysis conducted using PRISMA, which is essentially the gold standard for research. They searched databases like PubMed, ScienceDirect, and Springer for papers published up to June 2019. They were looking for keywords like ADHD, time perception, timing, time deficit, time reproduction, estimation, and discrimination. After gathering everything and excluding irrelevant papers, they ended up with 25 papers that met the criteria.
William Curb: It’s also worth mentioning that they excluded any studies involving comorbidities other than ADHD. This makes the findings very specific to ADHD, rather than ADHD combined with something like depression.
Skye Waterson: That’s a really good point. Depression, for example, can also cause issues with time—like feeling like you've been sitting for hours when it’s only been minutes. By narrowing it down, the results are much cleaner.
In terms of what they found: there was a significant impairment in time estimation, reproduction, discrimination, anticipation, and motor tapping in children with ADHD across at least one domain and one time interval. This meta-analysis confirms what other papers have found: we struggle with time pretty much globally. There was another paper I recently spoke about where they found that even if someone was on medication that supported dopamine and working memory, they still struggled with time. This is a core part of the ADHD experience.
William Curb: It’s something that really needs to be monitored because we develop so many "hacks" to deal with it. I was talking to someone yesterday about how a lot of our time blindness is solved through anxiety. It’s a very unhealthy coping mechanism, but it’s effective.
Skye Waterson: Usually, if I ask a client how they are managing, anxiety is the main tool they are using.
William Curb: When you step away from that and realize, "Oh, I’m not checking the clock every two minutes," it actually helps your focus. You aren't dealing with that constant anxiety of time blindness, so you can actually engage in what you’re doing.
Skye Waterson: It reminds me of a conversation I had recently with Jenna Free about the nervous system and how it affects ADHD management. But at the end of the day, we know that compared to neurotypicals, we struggle with this significantly more.
William Curb: Absolutely. One of the funny things about reading this paper was thinking, "This is really interesting," and then also, "Yep, I know all of this. This is how I’ve lived my whole life." But we need this kind of research to build a foundation. We need it confirmed scientifically so we can move beyond just talking about it colloquially.
Skye Waterson: Exactly. The goal of research is to indicate that these things are likely true across the board. That’s how these concepts finally enter the zeitgeist.
William Curb: What’s interesting about this study is that they looked at specific durations. I mentioned that "five-second mark" earlier—the idea that tasks longer than five seconds cause performance to drop significantly. How do you feel about that lab-based finding?
Skye Waterson: It’s interesting, but there is always a question of how things translate from the lab to the natural environment. In a lab, they might find that a three-second estimation is accurate, but a twelve-second one is way off. In the real world, we are talking about much larger gaps.
William Curb: I was thinking about this while playing a video game. Some powers take two or three seconds to recharge, and I can time those perfectly. But I have other powers that take 90 seconds to return, and that feels like an eternity. I’ll completely forget about it, and then suddenly it's back and I'm surprised.
Skye Waterson: That is such a great real-life example! I often describe this to the CEOs I work with by saying their life is divided into "things due today," "things due by the end of the week," and "everything else." Most people with ADHD can feel the difference between today and tomorrow, but if I ask, "Can you feel the difference between three months and six months from now?" almost everyone says no. Three months or six months—it’s all just "the future." The longer the time horizon, the more we struggle.
William Curb: Those time horizons feel so far away when you are dealing with the "now."
Skye Waterson: And in real life, things get messy because of that anxiety we mentioned. People feel like, "I have to do this today or something bad will happen," but they don't have that same negative association with things further out, so those tasks don't feel urgent. We get locked into a cycle of urgency versus importance.
William Curb: Right. We focus so much on the urgent "now" that we lose sight of general importance, like exercise. It’s important in general, but since what I do today doesn't have an immediate consequence, it gets lost.
Skye Waterson: It’s a lot to work through—learning how to feed those long-term tasks into your "today" view in a way you can actually manage. But knowing that we specifically struggle with longer time horizons has been very practically useful for the people I work with.
William Curb: That’s the big takeaway. Time blindness isn’t just a "sensory glitch" or a behavioral choice. It is a structural piece of executive function that we simply cannot do as well as neurotypical people. We have to keep that in mind and externalize our time monitoring with clocks and timers, because our internal clock just doesn’t work the same way.
